Phased Retirement Forms Now Available

As noted in Phased Retirement : Will It Be a Reality in Your Agency?, the first day to apply for phased retirement is November 6th.

OPM has previously indicated it would make the necessary forms to apply for phased retirement available by November 6. The forms were released in their final form on this date. Here are copies of the forms you may need in applying for the new phased retirement program:

SF 2825 Information & Instructions for Completing an Application for Phased Retirement under the Civil Service Retirement System (CSRS) (PDF file) [650.29 KB]
SF 2826 Information & Instructions for Completing an Application for Full Retirement Status (from Phased Retirement) under the Civil Service Retirement System (CSRS) (PDF file) [653.47 KB]
SF 3117 Information & Instructions for Completing an Application for Phased Retirement under the Federal Employees Retirement System (FERS) (PDF file) [641.45 KB]
SF 3118 Information & Instructions for Completing an Application for Full Retirement Status (from Phased Retirement) under the Federal Employees Retirement System (FERS) (PDF file) [614.05 KB]

Also, note that phased retirement is not yet available in many agencies and that implementing the program at all is largely at the discretion of each agency. Readers will need to check with their human resources office to find out about current availability of this program or when it may be available.
