Although your statements make sense as written, the actions taken based on your statements are nonsense.

Counselor should listen. This is true, but the counselor should do more than listen. Counselor should consider, weigh in on the statements and respond accordingly. The manager I know listens and ignores the comments or makes such stupid responses so that this person has a one track mind. For example, if a fact is not listed in one area, but is substantiated in another area, the manager states that the manager is only looking in the one area. The manager writes the "facts" accordingly.
Another example is certain mitigating facts stated by the worker that are not listed in the couseling memo. These are not stated in the counseling memo.

Another problem is that the manager cites other workers without naming them. Who knows if these cites are true or not.

The list is good if followed in spirit and practice.
